Notification of Maternal Deaths

The new format of Maternal death review has started on 1st January 2021.  The present format has two arms.

  1. Strengthened institutional review for which an Observer from SLCOG and FHB would participate. Here, a red flag could be raised by participants other than the observers, which could initiate an early action meeting with either DGHS or representative of him.
  2. Confidential enquiry leading to a national-level assessment of causes and recommendations for corrective actions.

Attached is the incidental report form for maternal deaths.

Kindly fill the attached E format of the incident form within 48 hours of maternal death and email it to SLCOG.
